Job Description: As a Nurse Practitioner in Internal Medicine at SSM Health St. Mary's Hospital in St. Louis, Missouri, you will play a key role in delivering high-quality primary care services to adult patients within an outpatient setting. You will collaborate with a multidisciplinary team to assess, diagnose, and manage a variety of acute and chronic medical conditions.

Key Responsibilities:

Patient Assessment and Diagnosis: Conduct thorough health assessments, including medical history reviews, physical examinations, and diagnostic tests to evaluate patients' health status and formulate accurate diagnoses.

Primary Care Services: Provide comprehensive primary care services to adult patients, focusing on health promotion, disease prevention, and management of chronic illnesses such as diabetes, hypertension, cardiovascular diseases, and respiratory conditions.

Treatment Planning and Management: Develop and implement individualized treatment plans based on evidence-based practices, including prescribing medications, ordering and interpreting diagnostic tests, and coordinating referrals to specialists as needed.

Patient Education and Counseling: Educate patients and families about health conditions, treatment options, preventive care, and lifestyle modifications. Empower patients to actively participate in their healthcare decisions and self-management strategies.

Collaborative Care: Collaborate with physicians, nurses, pharmacists, and other healthcare professionals to ensure coordinated and integrated patient care. Participate in interdisciplinary rounds, case conferences, and care coordination meetings to optimize patient outcomes.

Documentation and Record-Keeping: Maintain accurate and timely electronic health records (EHR) documentation of patient encounters, treatment plans, and follow-up care in compliance with regulatory standards and organizational policies.

Quality Improvement Initiatives: Participate in quality improvement projects and initiatives to enhance the quality and safety of patient care, promote clinical excellence, and achieve organizational goals.

Qualifications:

  • Master's degree in Nursing from an accredited program
  • Current licensure as a Nurse Practitioner in Missouri with national certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP)
  • Minimum of 2 years of experience practicing as a Nurse Practitioner in internal medicine or a related specialty preferred
  • Strong clinical assessment, diagnostic, and therapeutic skills
  • Excellent interpersonal and communication skills
  • Ability to work independently and collaboratively within a multidisciplinary healthcare team
